Forms 

There are two different forms for making a declaration of insider holdings: Form 1 is used for filing a basic declaration and Form 2 for filing a declaration of holdings and a declaration of changes in holdings.

A basic declaration is filed containing data on the insider himself, persons under his guardianship, spouse or cohabitation partner, other members of the same household and corporations controlled by them and corporations in which they exercise influence.

A declaration of changes is filed when information declared in the basic declaration changes.

A declaration of holdings is filed by the insider giving details of both the insider's own holdings of securities and persons closely associated with him. The declaration must be filed when the disclosure obligation becomes applicable. No declaration is required in respect of holdings in securities that are maintained under the book-entry system.

Changes in the holdings of securities are reported in the declaration of changes in holdings. No declaration is required in respect of holdings in securities that are maintained under the book-entry system.
